Healthcare Group of Arizona (HCG) uses the strength and diversity of several contracted provider networks to help its members find the benefit plans and providers that fit their needs. Care1st Health Plan Arizona, Mercy Healthcare Group, and University Physicians Healthcare Group provide HCG’s Classic Healthstyles, Secure Advantage Healthstyles, and Active Healthstyles benefit plans through their provider networks. The Arizona Foundation for Medical Care’s PPOS provider networks are used for HCG’s Consumer Choice PPOS benefit options. The Holman Frazier Behavioral Health provider network is also used for the PPOS benefit options. HCG offers dental plans through Employers Dental Services and The Principal Life Insurance Company. HCG is contracted with Avesis for its vision plan.
