How to Enroll your Small Business
Thank you for selecting Healthcare Group as your preferred healthcare provider. After you have reviewed the available plans and rates, please follow these easy steps to enroll you and your eligible employees in a Healthcare Group benefit option plan.
Step 1: Download the enrollment materials and review the forms.
Step 2: Although all forms, benefit
options, and premium rates are available to you in the enrollment packet
and the HCG web site, all new-enrolling groups are encouraged to speak
to a HCG enrollment specialist who can help you understand the
eligibility requirements and the documents needed to enroll. Please call
602.417.6755 (800.247.2289 outside Maricopa County) to schedule an
appointment.
Step 3: Complete the applicable forms in the enrollment packet.
Step 4: Return all required forms and your payment by the 15th of the month to ensure your effective date for the first of the following month.
You will receive your Member ID cards and handbook within 2-3 weeks of processing your enrollment paperwork.
